Using your military transcripts to transfer credit.

Each of the military services has developed methods for documenting military training that the American Council on Education (ACE) has evaluated for college credit and that you may be able to apply to your University of Phoenix degree program. For information on obtaining your military transcript, simply select your service below.

Army

The Army uses the Army/American Council on Education Registry Transcript Service (AARTS) for documenting military training. If you are interested in obtaining a personal copy for a pre-evaluation, you can print one from the following Web site: http://aarts.army.mil/.

If you are interested in obtaining an “institutional copy” of your AARTS transcript for admission to University of Phoenix, you must complete a DA Form 5454-R, Request for Army/American Council on Education Registry Transcript, which can also be obtained from the Web site listed above.

Once completed, the form should be faxed to (913) 684-2011, DSN 552-2011, or mailed to:

Navy/Marines

The Navy uses the Sailor/Marine American Council on Education Registry Transcript (SMART) for documenting military training for all active duty Sailors/Marines, and for those that have separated or retired after October 1, 1999. If you are not eligible for a SMART, you must submit your DD Form 214, Certificate of Release or Discharge from Active Duty, or a DD Form 295, Application for Evaluation of Learning Experiences during Military Service, to obtain credit for your military training.

If you are interested in obtaining a personal copy for a pre-evaluation, you can print one from the following Web site: https://www.navycollege.navy.mil/transcript.html.

You may request an “institutional copy” of your SMART for admission to University of Phoenix through the installations Navy College Office or Marine Corps Education Center. You can also download the request form from the Web site listed above. The fax number and/or address for mailing the request is contained on the form.

Air Force

The Air Force uses the Community College of the Air Force (CCAF) transcript for documenting all college credit–worthy training received. Air Force enlisted personnel should complete an AF Form 2099, Request for Community College of the Air Force Transcript, at the base education office, and have the transcript sent directly to their University of Phoenix Military Enrollment Representative (transcripts sent directly to students are unofficial copies and will be stamped “Issued to Student”).

If you are not located near an Air Force Base, you can send a written transcript request including your full name, Social Security Account Number, current address including ZIP Code, and your Military Enrollment Representative’s address. The letter must include your original signature, because that is the legal authorization for CCAF to release a transcript.

Since the Southern Association of Colleges and Schools (SACS) regionally accredit CCAF, the Office of Admissions will evaluate the transcript just like any other transfer credits would be evaluated. For more information, contact the CCAF Web site: www.au.af.mil/au/ccaf.

Coast Guard

The Coast Guard uses the Coast Guard Transcript for documenting all college credit–worthy training received. Before you may request a Coast Guard Transcript, you must first complete an Application for Voluntary Education Services (CGI 1561). A copy of this form is available from the following Web site:

Once at the site, click on the link above. You’ll need Adobe Acrobat Reader to read and fill in the forms.

If you've previously accomplished an Application for Voluntary Education Services (CGI 1561) then you can click on the "Official U.S. Coast Guard Transcript Request" and fill out CGI Form 1564 to obtain your official transcript.

Once completed and printed, the forms can be sent to the Coast Guard Institute by (1)clicking on the submit button, (2)printing and mailing a hard copy or(3)by fax:

The official Coast Guard Transcript, printed on blue paper, must be mailed to your  University of Phoenix Military Enrollment Representative’s address.
